Each entry should briefly explain what changed, why, and who approved it.
How to Log Changes
When a policy is updated:
Add a new entry below the relevant policy heading.
Use the template format shown below.
Keep descriptions factual, not narrative (e.g., “Added Confluence → Jira reference”).
Always include the approval source (Board, SMF 16, or minor edit).
