This page provides a record of all changes made to policies and governance documents after publication.

Each entry should briefly explain what changed, why, and who approved it.


How to Log Changes

When a policy is updated:

  1. Add a new entry below the relevant policy heading.

  2. Use the template format shown below.

  3. Keep descriptions factual, not narrative (e.g., “Added Confluence → Jira reference”).

  4. Always include the approval source (Board, SMF 16, or minor edit).

  • Minor grammatical or formatting changes can be logged as Editorial Update.
